OSHA initiated an inspection in response to a formal complaint and cited six willful and eight serious violations. Alleged willful violations include failure to provide adequate energy control procedures and a hearing conservation program. The serious violations allege a lack of machine guarding, failure to conduct a hazard assessment of the workplace to determine the need for PPE, failure to provide PPE for workers and failure to provide and use PPE when working on or near energized electrical equipment. Other-than-serious violations were cited for alleged recordkeeping issues. [Chapman Printing Co., Huntington, WV, Dec. 18, 2009].
