An inspection under OSHA’s national emphasis program for petroleum refineries resulted in citations for numerous allegedly serious safety violations, including failure to provide process safety information for pressure vessels; failure to sufficiently develop a written mechanical integrity program; failure to conduct regularly scheduled inspections and failure to designate hazardous classified locations, conduct compliance audits and maintain an audible alarm system.
