A supervisor at a ship repair dry-dock was crushed to death by a large plate of steel as it was being lowered. He and his crew had been repairing the bottom of a ship. They had cut around the damaged area and were preparing to lower a piece of steel. The victim was overseeing the operation and gave the all-clear signal.
