OSHA opened an investigation after a six-ton piece of stainless steel became dislodged from a crane and struck an employee at a Pennsylvania manufacturing facility. As a result, 42 serious and four repeat violations were cited. Serious violations include alleged failure to ensure that a swivel hook on an overhead crane was equipped with a safety latch and alleged operation of an overhead crane by workers who were insufficiently trained.
