Fifty-eight serious and five repeat violations were cited for workplace hazards that allegedly put employees at risk for cuts, crushing injuries, amputations, falls, electrocution, asphyxiation, burns and struck-by injuries. Repeat citations allege water-covered floors, lockout/tagout issues, unlabeled electrical devices and failure to remove defective forklifts from service.
