A 39-year-old father of five was killed when he was run over by a dump truck. He was a truck driver with 12 years of experience and had been with his employer for more than two years. There were two single-bed dump trucks and a front-end loader on site. The loader operator filled the back of one of the trucks, told the victim that he could leave and observed the victim getting into the cab of the truck.
